Register for Run Club!

WAVIER: n consideration of participation in LYTE RUN CLUB, the undersigned, on his or her behalf and on behalf of the minor child or children identified herein, hereby acknowledges and agrees to the following:

-that I am the parent or legal guardian of the minor child/children and I consent to the minor child's/children's participation in club

-that I acknowledge that there are risks associated with participating in the club including, but not limited to, severe physical injury resulting from participating in club activities; that I fully understand and will instruct the minor child/children that certain activities may involve inherent danger as well as risk to bodily injury and/or personal property damage or loss;

-that I understand and acknowledge that I am fully aware of and assume the risks (including but not limited to the risk of serious bodily injury as well as property loss or damage) of said minor child's/children's participation;

-that I understand that LYTE RUN CLUB ("LYTE"), its owners, officers, directors, volunteers, employees, insurers, and agents shall have no responsibility to pay

for medical treatment and related costs if said minor child/children is/are injured;

-that knowing the risks described above, I agree, personally and on behalf of the minor child/children identified herein, to assume all the risks and responsibilities surrounding the minor child's/children's participation in club;

- that, to the fullest extent allowed by law, I hold harmless and agree to indemnify LYTE, and their officers, directors, volunteers, employees, insurers, and

agents from and against any present or future claim, cause of action, loss or liability for injury to person or property, which said minor child/children may suffer or for which said minor child/children may be liable to any other person, related to said minor child's/children's participation in activities resulting from any cause whatsoever, and regardless of fault; and that I understand and agree that no oral or written representations can or will alter the contents of this document, and further, I agree that this agreement shall be governed by the laws of the State of Maryland (excluding its conflict of laws principles).

LATE POLICY- Children must be picked up promptly after the end of each session. There is a 10 minute grace period allotted. A fee of $1 per minute will be charged for any child not picked up after the 10 minute grace period. This fee must be paid prior to your child attending the next class.

REFUND POLICY- refunds will only be issued if LYTE initiates the refund due to program circumstances. NO OTHER refunds will be provided.

WEATHER: We will attempt to run in all conditions, excluding thunder and lightening; if weather seems extreme we will reach out to parents at least one hour before the class start time. Each session has ONE built-in weather make up session.

PHOTO RELEASE: I consent the LYTE Run Club to use of any photographs or video recording that are taken of my child while participating in the activity for use in Company brochures and program materials that are distributed both as printed document and on the internet. No payment will be made for use of these photographs and/or videos. Your child's name would never be used in connection with these images. *

BEHAVIOR: LYTE Run Club reserves the right to release, without refund of program fees, any child whose behavior becomes unmanageable.

COVID-19: LYTE Run Club has put in place preventative measures to help reduce the spread of COVID 19. By signing this document and registering my child(ren) I agree that I will NOT send my child(ren) to run club if myself or anyone in my household have been experiencing any of the following symptoms within the last 14 days:

    * Fever at or above 100.4 degrees Fahrenheit

    *Persistent dry cough

    *Chills

    *Sore Throat or loss of taste or smell

    *Muscle pain or headache

    *Runny Nose

I also understand that LYTE Run Club, LLC cannot be held liable for any exposure to the COVID 19 virus caused by misinformation or health history provided by each client. *MASK OPTIONAL*
